Job Summary
Evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Administer diagnostic tests and therapeutic treatments, including audio logic screening, while assessing disorders of speech, voice, language, hearing, and swallowing. Develop and review Plans of Treatment and Care, instruct patients and families on alternative communication systems, and participate in care coordination and discharge planning. Maintain documentation, submit reports, and observe patient condition changes to the supervisor and physician. Maintain confidentiality and adhere to CenterWell Home Health's professional standards. Full-time 40-hour schedule with a pay range of $85,400 - $117,500 annually. Requires current unrestricted license, CPR certification, and valid driver's license.
Required Qualifications
- Minimum six months experience as a speech therapist / speech language pathologist
- Current and unrestricted license
- Current CPR certification
- A valid driver's license
- Auto insurance
- Reliable transportation
- TB screening
- Maintain personal vehicle liability insurance
- Vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
