Home Health Speech Language Pathologist

Conduct in-home evaluations and treatment for adult and senior patients, assessing speech, language, cognition, and swallowing disorders. Develop and implement individualized care plans while delivering evidence-based therapies directly in patients' homes. Collaborate closely with an interdisciplinary team to educate patients and caregivers, document visits accurately, and support safe, independent living.
