Home Health Speech Language Pathologist

Conduct in-home evaluations and treatment for adult and senior patients, assessing speech, language, cognition, and swallowing disorders. Develop and implement individualized care plans while delivering evidence-based therapies in patients' homes. Collaborate closely with an interdisciplinary team, educate patients and caregivers, and document visits accurately to support safe, independent living.
