Conduct in-home evaluations and treatment for adults with communication, cognitive, and swallowing disorders. Develop and implement individualized care plans while delivering evidence-based therapy to maximize patient safety, autonomy, and quality of life. Educate patients and caregivers on care strategies. Collaborate closely with an interdisciplinary team to document care in EMR, ensure regulatory compliance, and contribute to continuous quality improvement within a culture of integrity and patient-centered care.
