Home Health Speech Language Pathologist

Conduct home health visits in Marlborough, Shrewsbury, Framingham, Hopkinton, Grafton, and surrounding towns to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties. Develop treatment plans based on evaluations and physician orders, then continually revise care plans while collaborating with multidisciplinary clinicians. Educate patients and family members on rehabilitative activities to promote independent living, and accurately document all observations and interventions using a touch pad tablet. Prior home health experience is preferred.
