Job Summary
Provide psychosocial and anticipatory grief counseling to patients, families, and caregivers in a Home Health/Hospice setting; perform initial psychosocial and bereavement risk assessments within five days of Hospice election; refer patients and families to community resources and financial assistance; assist with placement in facilities for basic care or respite; contribute to plan of care, participate in IDT meetings, and help with discharge planning; maintain documentation; mentor colleagues and assist with training; educate patients and families about disease progression and self-care techniques.
Required Qualifications
- Master's degree in social work from a CSWE-accredited program
- Active Ohio Social Work license
- Valid driver’s license and reliable transportation
- Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness and dying
