Job Summary
Assess psychosocial status of patients, families, and caregivers regarding terminal illness and environment, communicating findings to the interdisciplinary group. Conduct social evaluations covering family dynamics, caregiver abilities, and suicide risks to plan interventions and provide crisis counseling. Educate the team on cultural needs and assist patients in preparing advanced directives while serving as a liaison to community agencies. Maintain clinical records, participate in quality improvement teams, and assist in planning funeral and legal arrangements. Requires MSW with home health experience; offers PRN hours in Indianapolis with competitive wages and benefits including 401(k) and tuition assistance.
Required Qualifications
- Master of Social Work (MSW)
- home health experience
- graduate of a Master's program in Social Work accredited by the Council on Social Work Education
- Minimum of one (1) years' experience in health care
- Understands hospice philosophy, and issues of death/dying
- good verbal and written communication
- organization skills
- current CPR Certification
