Home Health Scheduler

Scheduling Home Health visits in coordination with nurses and clinical staff; maintain and update clinician schedules for efficient coverage; communicate scheduling needs with patients, families, and clinical staff; assist with visit frequency management per physician orders and plan of care; ensure visits comply with company policies and regulatory requirements; collaborate with clinical leadership to optimize scheduling efficiency; perform general administrative and office support duties; requires strong organizational, communication, and multitasking skills and proficiency with EMR systems and Microsoft Office.
