Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers and complete timely, accurate documentation supporting quality and compliance. Work Monday through Friday full-time with shared on-call rotation in the Prior Lake, Lakeville, and surrounding Minnesota territory. Requires an active RN license with one year of clinical experience, current driver's license, and CPR certification.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
- Has auto insurance
- Is CPR certified
- able to obtain certification for CPR
- must comply with all health-related requirements in all relevant jurisdictions
