Job Summary
Deliver individualized, one-on-one nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide skilled interventions including wound care, infusions, and medication management while educating patients and caregivers on symptom management. Monitor patient progress closely, communicate with physicians to adjust care plans, and coordinate continuity across providers. Complete timely, accurate documentation reflecting the full clinical picture. Schedule includes Monday through Friday full-time visits with shared on-call rotation. Requires active RN license with one year of clinical experience, current driver's license, and CPR certification.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
