Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate and empower patients and caregivers through clear instruction, while monitoring progress closely to communicate effectively with physicians and adjust plans to prevent avoidable hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality and compliance. Serve as the primary clinical partner for patients and families in the Sauk, Lodi, Merrimac, Poynette, Portage, Rio, Arlington, and Baraboo territories.
