Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, and medication management while educating patients and caregivers on symptom management. Monitor patient progress, communicate with physicians and the interdisciplinary team to adjust plans, and coordinate care across providers to ensure continuity. Complete timely, accurate documentation reflecting the full clinical picture. Requires an active RN license with one year of experience, CPR certification, and a current driver's license. Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Is patient-focused, organized, and motivated by delivering high-quality care
- Has a current driver's license, reliable transportation, and auto insurance
- Is CPR certified (or able to obtain certification)
- Comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
