Home Health Salaried Registered Nurse Case Manager Full Time

Deliver individualized, one-on-one nursing care in the home to promote patient recovery, safety, and independence. Conduct comprehensive in-home assessments evaluating physical, emotional, and environmental needs, then translate findings into patient-centered care plans. Provide skilled interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom management and daily care while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers to ensure continuity and complete accurate documentation supporting quality outcomes.
