Home Health Salaried or Per Point Occupational Therapist Full Time 15K Sign On Bonus

Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations and modify discharge care plans through timely documentation that ensures compliance with physician orders and payer requirements. Collaborate with interdisciplinary team members, physicians, and families to optimize patient well-being and coordinate services. Supervise other personnel as applicable and complete OASIS assessments where permitted.
