Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver occupational therapy services per physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, maintain documentation to ensure payer compliance, and coordinate discharge planning with healthcare teams and family members. Participate in interdisciplinary collaboration, supervise other personnel as applicable, and complete OASIS assessments where permitted. Work Mon-Fri days with flexible visit scheduling and no weekend requirement in the Viroqua, Westby, and Coon Valley territories.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
