Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations and discharge plans, collaborating with interdisciplinary team members, physicians, and family to optimize patient well-being. Maintain accurate documentation to ensure compliance with physician orders and payer requirements, including OASIS assessments where applicable. Supervise other personnel as needed and serve as a clinical resource consultant. Mon-Fri days with flexible visit scheduling, no weekend requirement, located in Viroqua, Westby, and Coon Valley. $15,000 sign-on bonus.
