Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Administer, evaluate, and interpret tests to determine functional goals, then provide written evaluations defining therapy needs, progress, and expected outcomes. Deliver occupational therapy services per physician orders while maintaining plans of care through timely documentation, goal modification, and discharge planning. Collaborate with interdisciplinary team members, physicians, and caregivers to optimize patient well-being and coordinate services. Supervise other personnel as applicable and complete OASIS assessments where permitted. Work Mon-Fri days with flexible visit scheduling and no weekend requirement in the Viroqua, Westby, and Coon Valley territories.
