Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver occupational therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining needs, goals, progress, and expected outcomes. Maintain plans of care through timely documentation, modifying goals and treatment as needed, and determining discharge care plans. Collaborate with interdisciplinary team members, physicians, nurses, and families to optimize patient well-being and coordinate services. Supervise other personnel where applicable and complete OASIS assessments as allowed by state regulations.
