Assess, teach, and deliver occupational therapy interventions directed by the physician while collaborating with interdisciplinary team members to optimize patient well-being. Perform initial and ongoing assessments of mobility, coordination, strength, and functional levels, administering tests to determine goals and outcomes. Provide written evaluations and plans of care, maintain documentation to prevent denials, and coordinate services with physicians, nurses, and families. Supervise other personnel as applicable and complete OASIS assessments where permitted. Work Mon-Fri days with flexible visit scheduling and no weekend requirement in the Viroqua, Westby, and Coon Valley territories.
