Assess, teach, and deliver therapy interventions directed by the Physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and coordinate services with interdisciplinary team members, physicians, and families. Maintain documentation to ensure compliance with physician orders and payer requirements, including OASIS assessments where permitted. Supervise other personnel as applicable and collaborate on discharge planning and home exercise education. Mon-Fri days with flexible scheduling, no weekend requirement, and a $15,000 sign-on bonus.
