Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Deliver occupational therapy services compliant with physician orders and maintain the plan of care through timely documentation, goal modification, and discharge planning. Collaborate with interdisciplinary team members, physicians, and caregivers to optimize patient well-being and coordinate services. Supervise other personnel as applicable and complete OASIS assessments where permitted. Work Mon-Fri days with flexible visit scheduling and no weekend requirement in Viroqua, Westby, and Coon Valley.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
