Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function. Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes. Provide written evaluations and plans of care defining occupational therapy needs, goals, progress, and expected outcomes. Deliver services to comply with prescribed physician orders while maintaining accurate documentation to prevent clinical denials. Collaborate with interdisciplinary team members, physicians, and caregivers to optimize patient well-being and coordinate discharge planning. Supervise other personnel as applicable and complete OASIS assessments where allowed by state regulations. Schedule includes Mon-Fri days with flexible visit scheduling and no weekend requirement. Territory covers Viroqua, Westby, and Coon Valley.
