Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels in home settings. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide written evaluations defining occupational therapy needs, goals, progress, and expected outcomes. Deliver therapy services compliant with prescribed physician orders while maintaining plans of care through timely documentation and modifying goals as needed. Collaborate with interdisciplinary team members, physicians, nurses, and caregivers to optimize patient well-being, coordinate discharge planning, and educate families on home exercise plans. Supervise other personnel including OTAs and HHAs where applicable, ensuring accurate OASIS assessments and documentation to prevent technical or clinical denials.
