Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations and discharge care plans, documenting client performance and modifying treatment as needed. Collaborate with interdisciplinary team members, physicians, and families to optimize patient well-being and ensure compliance with physician orders. Supervise other personnel and complete OASIS assessments where permitted.
