Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and coordinate services with the interdisciplinary team, nurses, and families. Maintain accurate documentation to prevent denials, supervise other personnel as applicable, and complete OASIS assessments where permitted. Work Mon-Fri days with flexible scheduling, no weekend requirement, and a $15,000 sign-on bonus in the Viroqua, Westby, and Coon Valley areas.
