Job Summary
Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations and modify treatment plans as needed, ensuring accurate documentation to prevent denials. Collaborate with interdisciplinary team members, physicians, and caregivers for discharge planning and education on home exercise programs. Supervise other personnel where applicable and complete OASIS assessments where permitted. Work Mon-Fri days with flexible scheduling, no weekend requirements, and a $15,000 sign-on bonus in the Viroqua, Westby, and Coon Valley territories.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
