Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations and discharge plans, collaborating with interdisciplinary team members, physicians, and caregivers to optimize patient well-being. Maintain accurate documentation to ensure compliance with payer requirements and supervise other personnel as applicable. Work Mon-Fri days with flexible visit scheduling and no weekend requirement in the Viroqua, Westby, and Coon Valley territories.
