Assess, teach, and deliver therapy interventions directed by the Physician within the home health plan of care. Provide written evaluations defining needs, goals, and outcomes while collaborating with interdisciplinary team members to optimize patient well-being. Maintain the plan of care through timely documentation, goal modification, and discharge planning that includes educating patients and caretakers on home exercise plans. Coordinate services with physicians, nurses, and families while ensuring documentation meets payer requirements to avoid denials. Requires a current state license, OASIS documentation experience, and Mon-Fri days schedule.
