Job Summary
Assess, teach, and deliver occupational therapy interventions directed by the physician to optimize patient well-being. Perform initial and ongoing assessments of mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify plans of care, and manage discharge planning by educating patients and caretakers on home exercise regimens. Collaborate with interdisciplinary team members, physicians, and nurses to coordinate services and maintain effective communication. Document client performance, interpret measurements, and complete OASIS assessments per policy to ensure compliance and minimize denials. Supervise other personnel as applicable. Mon-Fri days with flexible scheduling; no weekend requirement. Territory includes Viroqua, Westby, and Coon Valley.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
