Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver occupational therapy services, administer evaluations, and interpret tests to define goals and outcomes. Maintain the plan of care through timely documentation, modify treatments as needed, and create discharge care plans including home exercise education. Collaborate with interdisciplinary team members, physicians, and caregivers while supervising other personnel. Complete OASIS assessments and ensure documentation compliance to prevent denials. Work Mon-Fri days in the Viroqua, Westby, and Coon Valley territories with flexible visit scheduling and no weekend requirements.
