Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Provide written evaluations defining patient needs, goals, and expected outcomes while maintaining the plan through timely documentation and goal modification. Collaborate with interdisciplinary team members, nurses, and caregivers to optimize patient well-being and coordinate discharge planning, including educating families on home exercise plans. Ensure all services comply with prescribed physician orders and payer documentation standards to prevent denials. Maintain effective communication with staff and family while serving as a clinical resource consultant. Requires an accredited Occupational Therapy degree, current unrestricted state license, and experience with OASIS documentation.
