Assess, teach, and deliver occupational therapy interventions directed by the physician while collaborating with interdisciplinary team members to optimize patient well-being. Perform initial and ongoing assessments of mobility, coordination, strength, and functional levels, administering tests to determine goals and outcomes. Provide written evaluations and plans of care, maintain documentation to prevent denials, and manage discharge planning by educating patients and caretakers on home exercise plans. Supervise other personnel as applicable and complete OASIS assessments where permitted. Work Mon-Fri days with flexible visit scheduling in the Viroqua, Westby, and Coon Valley territories.
