Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations and discharge plans, collaborating with interdisciplinary team members to optimize patient well-being and coordinate services with physicians and families. Maintain documentation to ensure compliance with physician orders and payer requirements, supervise other personnel as applicable, and complete OASIS assessments where permitted. Work Mon-Fri days with flexible visit scheduling and no weekend requirement in the Viroqua, Westby, and Coon Valley territories.
