Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, maintain care plans through timely documentation, and collaborate with interdisciplinary team members to optimize patient well-being. Responsible for discharge planning, educating caretakers on home exercise plans, and supervising other personnel as applicable. Complete OASIS assessments where permitted and ensure documentation complies with payer requirements to prevent denials. Mon-Fri days with flexible visit scheduling; no weekend requirement.
