Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Deliver occupational therapy services per physician orders, maintain the plan of care through timely documentation, and modify treatment as needed. Provide written evaluations defining needs and expected outcomes, collaborate with interdisciplinary team members including nurses and physicians, and supervise other personnel. Complete OASIS assessments where permitted and coordinate discharge planning by educating patients and caretakers on home exercise plans. Work Mon-Fri days in the Viroqua, Westby, and Coon Valley territories with no weekend requirement.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
