Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to determine functional goals, then deliver occupational therapy services compliant with physician orders. Provide written evaluations and plans of care, modify treatment as needed, and manage discharge planning by educating patients and caretakers on home exercise plans. Collaborate with interdisciplinary team members, physicians, and nurses to optimize patient well-being, while maintaining effective communication and serving as a clinical resource consultant. Complete OASIS assessments where permitted and ensure documentation meets payer requirements to avoid denials. Supervise other personnel as applicable. Full-time Mon-Fri days with flexible visit scheduling and no weekend requirement.
