Assess, teach, and deliver therapy interventions directed by the Physician within the home health plan of care. Provide written evaluations defining needs, goals, and outcomes while maintaining plans through timely documentation and discharge care planning. Collaborate with interdisciplinary team members, physicians, nurses, and families to optimize patient well-being and coordinate services. Ensure all documentation complies with policy and payer requirements to prevent denials. Requires a degree in Occupational Therapy, current state license, and experience with OASIS documentation. Full-time Mon-Fri schedule with a $15,000 sign-on bonus.
