Job Summary
Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels, while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and manage discharge planning by educating patients and caretakers on home exercise regimens. Collaborate with interdisciplinary team members, nurses, and physicians to optimize patient well-being, maintain accurate OASIS documentation, and supervise other personnel as applicable. Work Mon-Fri days with flexible visit scheduling and no weekend requirements in the Viroqua, Westby, and Coon Valley territories.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons
