Assess, teach, and deliver occupational therapy interventions directed by the physician while collaborating with interdisciplinary team members to optimize patient well-being. Perform initial and ongoing assessments of mobility, coordination, and strength; administer tests to determine functional goals and outcomes. Provide written evaluations defining needs, goals, and expected outcomes, then modify plans and discharge care as appropriate. Maintain effective communication with staff, healthcare teams, and families while supervising other personnel where applicable. Complete OASIS assessments and document services to ensure compliance with physician orders and payer requirements. Work Mon-Fri days in Viroqua, Westby, and Coon Valley with a $15,000 sign-on bonus.
