Assess, teach, and deliver occupational therapy interventions directed by the physician to optimize patient well-being. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify plans of care, and document client performance to ensure compliance with physician orders and payer requirements. Collaborate with interdisciplinary team members, nurses, and families to coordinate services and educate caretakers on discharge home exercise plans. Supervise other personnel as applicable and complete OASIS assessments where permitted. Work Mon-Fri days with flexible visit scheduling and no weekend requirement in the Viroqua, Westby, and Coon Valley territories.
