Job Summary
Occupational Therapist (OT), salaried, full-time, providing in-home occupational therapy services in Wisconsin. Responsibilities include assessments of mobility, coordination, strength and function; development and documentation of plans of care; delivery of OT services per physician orders; discharge planning; OASIS (where allowed by state practice); coordination with healthcare team and family; supervision of OTAs/HHA as applicable; and participation in interdisciplinary care. Requires a degree in OT, active state license, and CPR. Preferred experience in home health therapy and OASIS documentation. Locations include Baraboo, Dells, Lake Delton, Portage, Lodi, Sauk, Merrimac, Pardeeville, Montello, Oxford, and Westfield (WI).
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program.
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
