Job Summary
Occupational Therapist (OT), salaried, full-time in Home Health across multiple Wisconsin communities. You will perform initial and ongoing assessments of mobility, coordination, strength, and function; deliver OT services per physician orders; develop and document evaluation and plan of care; discharge planning and home exercise guidance; participate in coordinating services with the healthcare team; supervise OTA/HHA as applicable; complete OASIS where allowed; and maintain thorough documentation and communication with staff and families. Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
