Job Summary
Occupational Therapist (Home Health) — Salaried, full-time position with Aveanna serving Winona, Wabasha, Buffalo, Trempealeau areas. Responsible for initial and ongoing OT assessments of mobility, coordination, strength, and function; development of written evaluation and plan of care; delivering OT services per physician orders; timely documentation; discharge planning and patient/family education; coordination with care team; supervising OTA/HHA as applicable; OASIS where allowed. Requires degree in OT, current state license in WI/MN, CPR and driver's license; preferred Home Health experience and OASIS proficiency. Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in WI and MN
- CPR certification
- Drivers License
- Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
