Job Summary
Administer skilled nursing care to patients on an intermittent basis in their place of residence under the direction of the Clinical Manager. Make initial evaluation visits to determine immediate care needs, assess eligibility and homebound status, and develop or revise the plan of care. Furnish services requiring substantial specialized nursing skill, initiate preventive and rehabilitative procedures, and counsel patients and families on nursing-related needs. Coordinate services with physicians, IDT members, and caregivers; inform personnel of condition changes and refer patients to PT, SLP, OT, or social workers as required. Supervise LPNs/LVNs and HHAs/CNAs, assign aides with written instructions, and conduct patient care conferences. Complete OASIS assessments, documentation, and discharge summaries in accordance with Agency policy while adhering to confidentiality standards. Take on-call duty nights, weekends, and holidays as assigned.
Required Qualifications
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) in which practicing
- Maintains current CPR certification
