Job Summary
Administer skilled nursing care to patients on an intermittent basis in their place of residence under the direction of the Clinical Manager. Make initial evaluation visits to determine care needs, assess homebound status, and develop or revise plans of care in coordination with IDT members. Furnish services requiring specialized nursing skill, initiate preventive and rehabilitative procedures, and counsel patients and families. Supervise LPNs/LVNs and HHAs/CNAs, assign aides to patients with written instructions, and conduct patient care conferences. Complete OASIS assessments, documentation, and discharge summaries in accordance with Agency policy. Take on-call duty nights, weekends, and holidays as assigned.
Required Qualifications
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) in which practicing
- Ability to exercise initiative and independent judgment
- Maintains current CPR certification
Desired Qualifications
- One (1) year nursing experience, preferred
