Job Summary
Administer skilled nursing care to patients on an intermittent basis in their place of residence in accordance with physician orders and the plan of care. Conduct initial evaluations, determine immediate care needs, and develop or revise care plans in coordination with interdisciplinary team members. Furnish services requiring specialized nursing skill, initiate preventive and rehabilitative procedures, and counsel patients and families on nursing-related needs. Supervise LPNs/LVNs and HHAs/CNAs, assign aides with written instructions, and conduct patient care conferences. Complete OASIS assessments, documentation, and discharge summaries per Agency policy, while reporting changes in condition and incidents to the Clinical Manager. Take on-call duty nights, weekends, and holidays as assigned.
Required Qualifications
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) in which practicing
- Maintains current CPR certification

- One (1) year nursing experience, preferred
