Job Summary
Administer skilled nursing care to patients on an intermittent basis in their place of residence in accordance with physician orders and plan of care under the direction of the Clinical Manager. Make initial evaluations to determine immediate care needs, assess eligibility and homebound status, and develop or revise the plan of care in coordination with IDT members. Furnish services requiring specialized nursing skill, initiate preventive and rehabilitative procedures, and counsel patients and families on nursing-related needs. Supervise LPNs/LVNs and HHAs/CNAs, assign aides to patients with written instructions, and conduct patient care conferences. Complete OASIS assessments, documentation, and discharge summaries while participating in peer review, quality management, and utilization review. Take on-call duty nights, weekends, and holidays as assigned.
Required Qualifications
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) in which practicing
- Maintains current CPR certification

- One (1) year nursing experience, preferred
