Home Health RN

Conduct home health nursing assessments to determine equipment needs, deliver and document interventions including medication administration and ordered tests, and instruct participants and families on disease processes and self-care techniques. Actively participate in interdisciplinary team meetings to develop care plans and collaborate with physicians to assist in smooth care transitions between settings. Track and monitor home health scheduling while maintaining a healthy and safe environment in participants' homes. Earn hourly pay including travel time with flexible scheduling and built-in documentation time. Requires an unencumbered RN license, one year of clinical experience with frail or elderly populations, and CPR/First Aid certification.
