Job Summary
Conduct comprehensive assessments on patients referred for home health nursing services and determine eligibility for admission. Formulate initial care plans according to the patient's Plan of Care, process admissions, and refer patients to other agency disciplines or community resources as needed. Provide ongoing assessments to determine service frequency, update care plans, and manage discharge in compliance with agency policy. Coordinate patient care by communicating with physicians, referral sources, and the multidisciplinary team both orally and in writing. Develop and implement teaching plans for patients and caregivers, maintaining clinical records and reports per agency guidelines. Work weekends in 12-hour shifts under the supervision of the Patient Care Manager and Clinical Coordinator.
Required Qualifications
- Registered Nurse license
- Baccalaureate degree
- Two years as a professional nurse in nursing practice
- Previous community health experience as a professional nurse
- Ability to communicate effectively, both orally and in writing
- Ability to exercise tact, initiative and good judgment
- Ability to honor and respect ethnic and cultural diversity
- Ability to occasionally lift and/or carry 100 lbs
- Ability to frequently lift and/or carry 50 lbs
- Ability to stand and/or walk at least 6 hours in an 8-hour work day
- Ability to stoop (bending at the waist) and crouch (stooping and bending at the knees)
- Ability to frequently handle (grasping)
- Ability to finger (fine manipulation)
- Finger and hand dexterity
- Visual and auditory acuity
- Ability to work under the supervision of the Patient Care Manager and Clinical Coordinator
- Ability to supervise licensed practical nurses and home health aides
- Ability to work weekends (Saturday and Sunday, 12-hour shifts)
