Job Summary
Provides home health nursing services in patients' residences with weekend 12-hour shifts. Collaborates with an interdisciplinary team to evaluate patients, develop and implement the Plan of Care, coordinate referrals to other disciplines, educate patients and families, maintain clinical records, and ensure compliance with agency policies. Demonstrates time-efficient home visits, teaching plans tailored to patient and caregiver needs, and effective communication with physicians, families, and the care team.
Required Qualifications
- Graduate of an approved school of professional nursing and licensed as a Registered Nurse in the practicing state
- Baccalaureate degree preferred
- Two years as a professional nurse in nursing practice or previous community health experience preferred
